Incidents

An incident in TPAS is a specific thing: a gate operator granted access by hand after a badge or an on-board unit failed.

It is not a complaint log or a maintenance queue. It exists so that every manual override has a name against it, a reason, and a trail — because a barrier that was opened without the system's approval is exactly the event an audit will ask about.

Log the incident at the time

morning. The record is only worth having if it is accurate, and by the end of a shift nobody remembers which card it was.

Who can do this

incident:read. A gate operator logs them (incident:create), a contractor sees their own organisation's, an administrator sees all. Insights → Incidents.

Card failures

Two tabs, because the two failures are investigated by different people:

Above the list: total incidents, how many are open, how many resolved. The list carries when it happened and the incident reference (INC-CARD-0001), the person, the card, the reason, the gate, the contractor and the status.
